Keeping Member and Associate information secure is most important to GetBenefitRelief. GetBenefitRelief will not share, sell or otherwise disclose information to any third parties not affiliated with GetBenefitRelief. The personal information provided is collected for the sole purpose of providing member, member dependents (if applicable) and associates with GetBenefitRelief services and benefits. We may disclose any of this information to our affiliates, service providers, third party service providers, provider networks and other suppliers of products and services to GetBenefitRelief members and associates. Personal information collected includes, but is not limited to: name, address, city, state, zip, telephone numbers, social security number, date of birth, gender, dependent name, dependent gender, dependent date of birth, dependent social security number, dependent relationship, beneficiary name, beneficiary data of birth, beneficiary gender, beneficiary relationship, bank account information, or credit card information. Personal information collected comes from one of the following: paper application, faxed application, and web application. GetBenefitRelief will maintain information even after the member cancels membership or associate is terminated/resigns from GetBenefitRelief. This information will be protected as that of an active member or associate. A member or associate may request in writing, after discontinuing the membership or resignation/termination, to remove the information from our file.

Dallas, TX 75244 • Telephone number of Discount Medical Plan Organization: 800-800-7616 • This discount card program contains a 30 day cancellation period. • This plan provides discounts at certain healthcare providers for medical services. • This plan does not make payments directly to the providers of medical services • The plan member is obligated to pay for all healthcare services but will receive a discount from those healthcare providers who have contracted with the discount plan organization

MD, ND, OK, SC, SD and TX residents: Member shall receive a full refund of membership fees, excluding registration fee, if membership is cancelled within the first 30 days after receipt of membership materials. • AR and TN residents: A refund of all fees will be issued if membership is cancelled within the first 30 days • The range of discounts for medical or ancillary services provided under the plan will vary depending on the type of provider and medical or ancillary service received. • The discount medical card program makes available, before purchase and upon request, a list of program providers, including the name, city, state, and specialty of each program provider located in the cardholder’s service area.
